Casa
offers immediate at home services including: all medications and
prescriptions that pertain to the illness; any medical
equipment needed including wheelchairs, walkers, hospital beds,
oxygen, etc.; 24 hour availability of experienced nursing care
helping with pain control and management, and symptom management
of physical problems that the disease process creates;
counseling for all concerned; physical and nutritional therapy
if appropriate; pastoral ministry; volunteer support; house
calls; social services; periodic around-the-clock nursing care
if necessary; intensive care in an inpatient unit to stabilize a
crisis or provide relief to a caregiver; and complete access to
any and all of modern technology that falls in line with the
hospice philosophy.
Casa de la Luz Hospice provides hospice care through an
interdisciplinary team of professionals and volunteers.
This team forms a community of care, with the patient and
family at its center. Our hospice care team is
available 24 hours a day, seven days a week. Our service
includes:

Hospice physician (or Medical Director) - for the medical
direction of the patient's care, provided by either the
patient's personal physician or a physician affiliated with
our hospice program

Hospice nurses - regular home care visits by registered
nurses and licensed; practical nurses to monitor the
patient's condition and to provide appropriate care and
maintain patient comfort;

Home health aide - attending to the patient's personal needs;
-
Chaplain services - for the patient and/or loved ones, if
desired;
-
Medical Social Worker - to assist with the emotional and
psychological stages of the terminal illness, and the
training and support of the caregiver;

Bereavement counseling - to help patients and their loved
ones with grief and loss;

Medical equipment
-
Medical supplies;
- Medications for symptom control and pain relief;
-
Volunteer support to enhance quality of life and help reduce
the burden of care giving

Physical, speech, and occupational therapy; and
-
Dietary counseling.

Eligibility:
The Medicare Hospice Benefit defines terminal as having a life expectancy of six months or less if the
disease runs its expected course. Hospice is available to
people who are no longer seeking curative treatments and who want to
focus on comfort and quality of life in the last six months of life.
Casa de la Luz Hospice is an advocate for people with end-stage illness
and their right to choose a program they are entitled to.
Evaluation:
Our staff has the expertise and dedication to care for people
with all diagnoses. We recognize that a prompt and
professional response to a referral is imperative; We have
intake and admission nurses available seven days a week.
The Casa Team is Quick to Respond:
- In the Hospital - We can help you plan for our hospice
service to begin upon your discharge to most any setting.
- At Home - We take care of your immediate needs first and
arrange scheduled visits by our hospice team.
- In Crisis - Our expert nursing staff answers your phone 24
hours a day to ensure that medically critical situations are
controlled.
- In a Nursing Home/Assisted Living Center - Our hospice
professionals work side by side with current care staff to
give you specialized attention. We provide all
entitled benefits, including medications, equipment, and
medical supplies.
- In Mourning - Our bereavement team provides up to 13 months of bereavement
support to the loved ones and friends you will leave behind.
