Volunteer Coordinator
The Volunteer Coordinator is responsible for planning, coordinating, and managing all program activities in the Casa de la Luz organization. The Volunteer Coordinator is responsible for development and maintenance of the volunteer orientation program certified by the Arizona Hospice and Palliative Care Organization. The VC must recruit, educate, and select volunteers; establish a need for volunteer services with the patient/family/caregiver and entire hospice team, supervise and evaluate volunteers in a timely and appropriate manner, document information for clinical records related to the volunteers, maintain monthly statistics, reevaluate the program as necessary, attend interdisciplinary group meetings, participates in budget preparation for program development, participates in organization-wide quality management program, etc...
Position requires: Bachelor degree, advanced degree in human services preferred. A minimum of two years experience in a health care setting, hospice and/or volunteer experience preferred. Must have the ability to supervise, coordinate and evaluate volunteer services, understand hospice philosophy and have the ability to organize and develop volunteer hospice personnel for patient and organizational needs. Must have the ability to recruit, manage and cultivate the program and volunteers. Requires excellent communication and organizational skills.

Admissions Supervisor
Admissions Supervisor is responsible for supervising all aspects of the patient intake and referral process including supervising members of the admission team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns and supervising the insurance verification and authorization processes and other related duties as assigned.
Requires, a current Arizona RN license, BS in Nursing, Masters Degree in health care or related field preferred. Prefer at least three to five years experience in health care management preferably in hospice care operations. Demonstrated ability to supervise and direct professional and administrative personnel. Ability to market aggressively and deal tactfully with customers and community. Knowledge of corporate business management, governmental regulations and private payor practices. Demonstrated excellent communications, negotiation and public relations skills. Demonstrated autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
RN Case Manager
RN Case manager is responsible for planning, organizing and directing hospice care. The Case Manager will complete an initial assessment of patient and family to determine hospice needs, provide a complete physical assessment and history of current and previous illnesses, write and initiate Plan of Care, and re-evaluate needs of patient/family/caregiver regularly and revise the Plan of Care as necessary. The Case Manager will administer medications and treatments as prescribed by the physician in the patient's Plan of Care, counsel the patient and family in meeting nursing and related needs, provide health care instructions to the patient as appropriate per assessment and Plan, and assist the patient with the activities of daily living, facilitating the patient's efforts toward self-sufficiency and comfort care. The Case Manager will work in cooperation with the family/caregiver and hospice interdisciplinary team members to meet the needs of the patient/family/caregiver.
Requires excellent assessment and problem solving skills, excellent oral and written communication skills, the ability to efficiently and effectively organize and prioritize. Ability to stand for long periods of time, bend, stoop and/or kneel, ability to lift, position and/or transfer patients, lift supplies and equipment.
Requires the candidate be a graduate of an accredited school of nursing, hold a current Arizona nursing license, current CPR certification, have 1-2 years previous experience in acute care setting and 1 year experience in hospice/home care setting.

Certified Caregiver for Residential Hospice Home
The Certified Caregiver is responsible for performing various services for a patient as necessary to meet the patient's personal needs and to promote comfort. These services include but are not limited to personal care, assuring patient safety, assisting with administration of medication, planning and preparing meals, laundry and housekeeping.
Requires excellent observation and problem solving skills, excellent oral and written communication skills, the ability to read and follow written instructions and document care given, the ability to stand for long periods of time, bend, stoop and/or kneel, and the ability to lift, position and/or transfer patients, lift supplies and equipment. Candidates must understand the hospice philosophy and be comfortable providing specialized care to the terminally ill.
Requires the candidate meet the training requirements of the State of Arizona (Directed Care, Supervisory Care, Personal Care), possess current CPR and First Aid certifications, and be 18 years of age or older.
Position is full time, 11 p.m. - 7 a.m., 40 hours per week
