Admissions Supervisor
Admissions Supervisor is responsible for supervising all aspects of the patient intake and referral process including supervising members of the admission team, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns and supervising the insurance verification and authorization processes and other related duties as assigned.

Requires, a current Arizona RN license, BS in Nursing, Masters Degree in health care or related field preferred. Prefer at least three to five years experience in health care management preferably in hospice care operations. Demonstrated ability to supervise and direct professional and administrative personnel. Ability to market aggressively and deal tactfully with customers and community. Knowledge of corporate business management, governmental regulations and private payor practices. Demonstrated excellent communications, negotiation and public relations skills. Demonstrated autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.



RN Case Manager
RN Case manager is responsible for planning, organizing and directing hospice care. The Case Manager will complete an initial assessment of patient and family to determine hospice needs, provide a complete physical assessment and history of current and previous illnesses, write and initiate Plan of Care, and re-evaluate needs of patient/family/caregiver regularly and revise the Plan of Care as necessary. The Case Manager will administer medications and treatments as prescribed by the physician in the patient's Plan of Care, counsel the patient and family in meeting nursing and related needs, provide health care instructions to the patient as appropriate per assessment and Plan, and assist the patient with the activities of daily living, facilitating the patient's efforts toward self-sufficiency and comfort care. The Case Manager will work in cooperation with the family/caregiver and hospice interdisciplinary team members to meet the needs of the patient/family/caregiver.

Requires excellent assessment and problem solving skills, excellent oral and written communication skills, the ability to efficiently and effectively organize and prioritize. Ability to stand for long periods of time, bend, stoop and/or kneel, ability to lift, position and/or transfer patients, lift supplies and equipment.

Requires the candidate be a graduate of an accredited school of nursing, hold a current Arizona nursing license, current CPR certification, have 1-2 years previous experience in acute care setting and 1 year experience in hospice/home care setting.

Position is full time



Receptionist
Receptionist is responsible for answering and appropriately routing incoming telephone calls, greeting visitors, distributing mail on a daily basis, and other miscellaneous clerical duties.

Requires excellent telephone skills, computer skills and a love of people. Must have the demonstrated ability to efficiently organize and prioritize, and the ability to work independently while functioning as a member of a team.

Requires high school diploma or equivalent. Position is M - F, 8 - 5, Bilingual preferred.

Maintenance Assistant
The Maintenance Assistant must enjoy physical and outdoor work. Must have experience with basic building/home repair work such as, drywall, painting, plumbing, irrigation, tile/grout repair, seasonal planting and landscape work, moving household/office equipment in and out of storage, etc... Must be a self starter, reliable, friendly, personable and non-smoker preferred. Ideal for semi-retired individual that enjoys physical work.

Position is part time, 24 hours per week, with potential for full time.
